TAX Information and Reminder
1095-C Statement Notice
Due to an update to the Paperwork Burden Reduction Act, ICLSD will no longer automatically mail 1095-C statements each year. Covered individuals and full-time employees may receive a copy of their 1095-C statement upon request.
